Records and Registration Office Forms

Students should consult the Academic Calendar for specific deadline dates to submit the following forms.

Schedule Adjustment - To be used only when registration options are not available on Web or Telephone Registration or when signatures are required for adding or dropping.

Special Processing Form - Use this form to register for Independent Study, Thesis, Practicum, Special Studies, Variable Credit, etc.

Military Waiver for Resident Tuition Form - This form is for active duty military personnel and their dependents requesting an adjustment to in-state tuition status.

Name and Number Change Form - Use this form to let the Records Office know of a name change or if your student number is incorrect.

Undergraduate Non-Degree Petition To Withdraw/Drop After Deadline Dates - This Form is to be used by undergraduate non-degree seeking students only. If you are in a degree program you must petition your dean directly.

Request to Receive the Academic Records of a Dependent Student - Parents who have the student listed as a dependent on their most recent income tax return need this form to receive a copy of the dependent student's academic record.

Request to Prevent Disclosure of Directory Information - Privacy Form, Allows the Records Office to put a privacy flag on your student records. Be careful, this can be an inconvenience if you are seeking employment. The Records Office cannot release your enrollment, degree status or major to anyone unless you come to the Records Office with a picture ID.

Transcript Request Form - Complete this form to have your CU-Denver transcript sent or faxed to another institution, employer, etc. Unofficial, student copies can be obtained at the Online Student Information site.

External Transcript Request - This form is used to request copies of the transcipts you sent to UCD from another institution. If you need official copies of your external transcipts, you need to request them from the institutions.

Interinstitutional Application - UCD students who want to take a course at Community College of Denver or Red Rocks Community College will need the registration form below.

Interinstitutional Registration - UCD students need this form to register for a course interinstituionally. You must complete the application above before submitting this form.

Faculty/Staff Tuition Waiver - Employees of the University of Colorado four campus system, Auraria, Community College of Denver and Metropolitain State College of Denver are eligible for up to six semester hours of tuition credit per fiscal year which begins summer semester. Continuing Education and Extended Studies courses including the SAVE program are excluded. You may complete the form online then print for appropriate signatures. Please read the Employee Waiver Information below before completing he form.
