USIEP Fellowship Application Checklist:

1.        2 copies of the USIEP Profile (Link to file); one hard copy and the other e-mail (see below). Both must be received by July 1, 2003.

2.        Application to CE PhD Program (Link to file) including:

§  academic record,

§  statement of purpose,

§  official transcripts and

§  three letters of recommendation.

Application must be received by July 1, 2003 (see below).

3.        Financial Need Assessment - students must complete the FAFSA (www.fafsa.ed.gov) and submit electronically to CU-Denver financial aid (www.cudenver.edu/finaid). For more information, contact Ellie.Miller@cudenver.edu, 303-556- 2325.

The USIEP profile should be completed and e-mailed to the Project Director (aramaswa@carbon.cudenver.edu); please include your name and the words "USIEP Profile" in the e-mail subject title. A hard copy must accompany your application.
